The Truth Mirage sash lock assembly (Amesbury Truth 45266 & 45376) is the forked handle and escutcheon cover used with concealed multi-point locks on casement windows. Forked handle measures 4-9/16" (115.9mm) and the escutcheon plate is 4-11/16" (119.1mm). Available in White or Bronze; link assembly is sold separately.

Truth Mirage Multi-Point Lock Handle and Escutcheon Assembly - 45266 / 45376
This is the Truth Mirage sash lock assembly used to operate concealed multi-point locks on casement windows. It includes the forked lever handle and the escutcheon (cover) plate that mounts over the lock body on the sash.
The forked handle engages an internal link assembly (sold separately) that drives the multi-point lock. Depending on your window, the correct link assembly length will vary, so confirm what is currently installed before ordering.
To access the internal link assembly, the wood side cover must be removed. The top and bottom trim pieces come off first, then the side cover can be lifted away.
